Important Note: TMRS forms require a signature and cannot be submitted electronically.Click on the form you want from the list below and fill it out on your computer. Then print out the form, sign it, and mail or fax the hardcopy to TMRS. If you print the form out and fill it in by hand, please type or use black ink. Also, please do not use highlighter on the forms.

Most Commonly Used TMRS Forms

1. New Member Enrollment (TMRS-0016). New members use this form when joining TMRS.
2. Address or Name Change Form (TMRS-CHNG). Members and retirees use this form to make name or address changes.
3. TMRS Change of Beneficiary for Vested Members (TMRS-007V). Use this form to name a beneficiary if you are a vested TMRS member (after five or 10 years of service, depending on your city).
4. Application for Restricted Prior Service Credit (TMRS-35R). If your city has adopted Restricted Prior Service Credit (see this page for a definition), use this form.
5. Service Retirement Packet (TMRS-SRP). This packet contains the forms needed to apply for a service retirement.
6. Direct Deposit Authorization Form. Use this form to establish or change your Direct Deposit authorization for payments from TMRS.
7. Refund Application (TMRS-0005). Members who leave TMRS-covered service and wish to apply for a refund of member deposits should use this form.
